Middle School Sports Physicals

Any students trying out for a sports team for the 2025-26 school year, will need a DIAA Physical dated after 4/1/25.     



You can get your physical completed at your doctor's office, at any walk-in clinic or in the Seaford Wellness Center.  
REQUIRED DIAA PPE PHYSICAL FORM
Completed sports physicals should be turned in to the main office at the Middle School.  .  PHYSICALS MUST BE TURNED IN OR YOU CANNOT PRACTICE THE FIRST DAY.
